What types of issue should be reported?
It is your obligation to report any activity that violates or appears to violate the law or Texas Children’s policies and procedure. Below are some examples of issues that should be immediately reported:
- Falsifying medical records
- Fraudulent billing
- Theft
- Lack of documentation to support claims
- Inappropriate access of a patient’s medical record
- Impermissible use and disclosure of protected health information
- Conflicts of interest
- Violation of applicable laws, regulations or policies
How do I report a concern?
You can raise your concern with your supervisor or another leader in the organization. If you don’t feel comfortable with this approach, you can also reach out directly to the Compliance & Privacy Office.
What if I want to make a report anonymously?
You can also report your concern anonymously:
- Call the Texas Children’s Confidential Hotline at 1-866-478-9070 (available 24 hours a day/7 days a week)

Can I be punished or otherwise retaliated against for reporting a concern?
Absolutely not. Texas Children’s is committed to open communication. Retaliation of any kind for reporting questionable practices in good faith is prohibited.
